An updated Emergency Release Information card is required for each student every school year.
If you have not completed an updated Emergency Release Information Card and submitted it to the school for this (2014-2015) school year please make sure that you do so as soon as possible. An updated form is required for each child at the start of each new school year. This form must be completed by the parent/guardian of the child named on the form. Please fill out all sections of the form except the area designated For School Use Only.
