Online Fact and Fees Registration

Online Fact and Fees Instructions

Available for any returning DD2 students.  New students enrolled after June 1st must complete the fact and fees process at OMS on 8/7/18 or 8/8/18 from 7 AM - 1 PM or 3 PM - 7 PM.

 

 

Family Living with Another Family/Relative - will need to complete the required documentation prior to receiving schedule.  See requirements here.

 

 

ALL steps must be completed in order to receive your student’s schedule. Schedules will be emailed beginning August 2, 2018.

 

1.  Visit the OMS payment site to pay for your fees. Please note a $3.95 convenience is charged by the payment site.

Save your receipt/payment confirmation. You will need it for Step 2.   Payment Site

Use Google Chrome or Mozilla Firefox for payment site.  Payment site will not work using Safari.

 

Students who received free lunch during 2017-2018 school year can skip to step 2.

 

 2.  Complete the Online Fact and Fees Verification information here.

Students who received free lunch use FL for the transaction/receipt #.

 

3.  Complete Lunch Application (click here) MUST be completed for all students. IMPORTANT -Save your email confirmation!

 

4.  You will receive an email from ckpayseur@gmail.com (within 2-5 days) to complete the documents listed below using Adobe Sign.

  • Meal Participation Form – Will need confirmation # from online Lunch Application.
  • Consent to Bill Medicaid Form.
