CBI Forms & Info for Families

Many OASIS families choose to support student learning by accessing the experience and expertise of Community Based Instructors (CBIs) and Community Based Facilities (CBFs). CBIs and CBFs can be an integral part of student learning and enrichment within a given course and allow students to explore subjects of interest in greater detail, or to develop more advanced skills. All courses must align with Washington State Learning Standards and support the student’s learning plan.

 

 

OASIS FAMILIES:

 

INSTRUCTIONAL SUPPORT FUND: The process for accessing your Instructional Support Fund is described in the OASIS Handbook. The annual amount is $1,800 per year for each full time student. (Note: K-8th grade students are full-time and enrolled at 100%. High school enrollment % depends on the course load).  Funds are posted to your account each month for 9 months, at the rate of $200 per month per full time student, September through May. Families planning to use Instructional Support Funds to pay June CBI (Community Based Instructor) expenses must “save” funds from previous months to cover any CBI costs in June.

 

 

 

 

Before starting your CBI class...

 

 

STEP ONE: Send to your OASIS teacher:

 

2016-17 Parent Agreement for CBI Class - Please submit this form even if you used the same CBI last year.  One form per CBI.  You can list all of your kids that are using the CBI on the same form. This form needs to be signed by your teacher before you begin your CBI class. Please do not send this form to the OASIS office manager.

 

2016-17 High Risk Documentation - OASIS Families need to fill out a ‘High-Risk’ form only if the CBI course is a high-risk activity. Please remember to include proof of insurance (copy of insurance card) with the High Risk form. (Examples: swimming, martial arts, etc. – please refer to the approved CBI/CBF list to see if a 'High-Risk' form is needed). This form (if applicable) also needs to be signed by your teacher before you begin your CBI class. Again, please do not send this form to the OASIS office manager.

 

 

 

STEP TWO:  Make sure the CBI is on the Approved OASIS Community Based Instructors & Facilities List.  We can't pay for classes unless they have all of their paperwork in and are approved! (Reminder: Private lessons will not be considered for funding.

 

If you'd like to use a CBI who is not approved, keep in mind that OASIS accepts applications for new CBIs & CBFs from August 15th until April 14th. Priority will be given to CBI applications in August and September. After that, the application process may take much longer.
